KVKK Application and Information Request Form

To exercise your rights under Article 11 of the Personal Data Protection Law No. 6698, submit an application containing the information below, together with documents proving your identity, in writing to 9222/3 Sokak No:8 Yazıbaşı, 35875 Torbalı / İzmir or by e-mail to [email protected].

Information the application must contain

Name, surname and, if the application is in writing, signature,
Turkish ID number for Turkish citizens; nationality, passport number or ID number (if any) for foreign nationals,
Residential or business address for notification,
E-mail address, telephone and fax number for notification, if any,
Subject of the request.

What you may request

To learn whether my personal data is processed.
To request information if my personal data has been processed.
To learn the purpose of processing and whether the data is used in line with it.
To know the third parties to whom my personal data is transferred.
To request correction of incomplete or inaccurate personal data.
To request deletion / destruction of my personal data.
To request that correction, deletion or destruction be notified to third parties the data was transferred to.
To object to a result arising against me from analysis by automated systems.
To claim compensation for damage suffered due to unlawful processing.

Handling of the application

Your application is concluded free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request. If the process requires an additional cost, the fee in the tariff set by the Personal Data Protection Board may be charged.
